Nandrolone decanoate and Sustanon are among the most commonly used, administered in cycles and supraphysiologically to enhance muscle gains, triggering damage resulting from oxidative stress and inflammation pathways. The liver is one of the most affected organs, with progressive structural and functional damage that can progress to fibrosis and ultimately compromise overall health. Therefore, the objective of this research was to evaluate the effects of supraphysiological and synergistic supplementation of two anabolic steroids in trained adolescent rats. The animals were divided into four groups: trained control rats (RT); nandrolone-treated rats (RTN); treated with Sustanon (RTS) and treated with nandrolone + sustanon (RTNS). Morphometric, histological, histochemical (Mallory's trichomycosis and P.A.S), oxidative stress (TBARS, GSH), inflammatory cytokines (IL-1β, TNF-α), biochemical markers for liver injury and metabolic, and organosomatic indices were analyzed. Animals treated with Sustanon showed central lobular congestion, hepatocellular ballooning, leukocyte infiltration, cord disorganization, collagen deposition, reduced glycogen, and biochemical alterations with increased LDL, ALT, AST, and decreased HDL. The Nandrolone group presented findings similar to the Sustanon group, with emphasis on steatosis and elevated liver injury markers. The Sustanon + Nandrolone combination intensified these effects, evidencing multigoticular steatosis and higher ALT and AST levels. Thus, the research seeks not only to expand scientific knowledge about the effects and risks of non-therapeutic use of these androgenic substances, but also, through the data, to help develop preventive measures and intervention strategies. By highlighting the potential health impacts, the aim is to provide information that contributes to public awareness, the work of healthcare professionals, and the development of public policies capable of assisting in social education and thus reducing the harms associated with this consumption.
